Once we receive physician orders for home health, we will contact you (or your designated caregiver) to schedule an appointment. A Bonita Springs healthcare professional will meet in your home for a comprehensive assessment of your health history and current medical needs. Once we have conducted our evaluation and answered any questions you may have, we will establish a care plan in conjunction with physician orders.
